Transitional Hospital to Home services including assistance with transfers, linking into and coordinating services, medication reviews, home safety assessments and equipment.

Hospital in the Home (HITH) services, enabling you to be discharged early from hospital or avoid hospital altogether. Services range from complex wound care and stoma care to intravenous antibiotic therapy, where we work closely with your doctors to provide high level clinical care at home.

Sleep Over Care helps you at bedtime and again in the morning with showering, dressing and breakfast, with help ‘on hand’ during the night in case you need it.

Active Night Care provides extra help at night when you are unwell.

Live-In Care is help ‘on hand’ around the clock, enabling you to remain in your own home even as your needs increase. Live-in care can be long term or short term, for example to provide respite for your primary carer or to assist in your recovery after hospitalisation.

Preventative Care includes a range of exercise programs and interventions that can help you maintain and improve your well-being at home. Vitalis also coordinates a range of allied health providers who provide services at home to ensure you get the best preventative care.
